* This also adds separate documentation, header, Fortran wrappers and
  pkg-config file for the cdi-pio part of the library

* This hopefully fixes a problem introduced with support for
  differential timesteps.

* The previous coding assumed that some non-zero amount of data
  would be written to every file opened.

* The "queue" data structure actually implements a set anyway and will
  be renamed subsequently.

* The "index" is now a member of the "enqueued" struct.
