changed processAddInputStream and processAddOutputstream to work with pstream pointers instead of IDs

fixed bug that caused a thread to try to lock a already locked mutex, moved all functions for pipe_t into pipe_t eliminating the need for pstream_t in pipe.cc and pipe.h, added pipe name and setting of it, changed the calls to the pipe functions to calling the memberfunction
